Dear Parents/Guardian,
Akron Children's Hospital will be offering a vaccine clinic through the School Based Health Center at EPMS and EPHS on March 21, 2024 for all students 11 years and older. These vaccines will be given at your child's school during school hours. If interested please print out the following forms and send both consents back to your child's school.
